Find the Dead Savage Orc in the Badlands and click to start the quest, then click the corpse again to get the item.
Emperor's Shoulders

Find the Dead Savage Orc in the Badlands underneath the bridge. To get there, go to the north side and jump on the rock to the left. Line your character directly with the bridge underneath and fall down to it. You will lose most your health but you should survive the fall.
